The Home Based Services (HB) program is designed to provide an integrated approach to working with families with a child or adolescent who has a serious emotional disturbance (SED) diagnosis. The HB program is also designed to ensure that children with (SED) conditions receive culturally competent treatment that is intensive, and clinically appropriate in amount, scope and duration. The services to be provided will be adjusted to meet the needs of the (SED) child/adolescent and their family to prevent out of home placement due to their behavior, emotional disturbance, substance use, mental illness and/or developmental disability. The HB program offers an array of services which include but are not limited to: Community Living Supports, Respite, Treatment Groups and referrals for additional groups within the community.
Objective: To provide comprehensive services addressing mental health and substance abuse issues that will promote normal development and healthy family functioning, support and preserve families, reunite families who have been separated, and reduce the usage of, or shorten the length of stay in psychiatric hospitals and other substitute settings.
Eligibility: Children, Adolescents and families residing in Kent County.
Funding: Funding is provided by Network 180, the Heart of West Michigan United Way, and the generosity of corporate donors and private individuals.
