Seeking a case manager working in the Muskegon area with a substance use population.
Position works as an advocate and coordinator for individuals served. Responsibilities include developing a comprehensive support plan; identifying, securing, and coordinating community resources for the clients served, coordinating with other providers, addressing basic needs, assisting in development of independent skills, reviewing and monitoring program activity, and leading skill development groups. Successful candidates will be a team player, have excellent communication and organizational skills, and be knowledgeable about community services and resources.
Completion of a Bachelor’s degree in social work or related field, experience working with diverse populations, a valid Driver’s License, and reliable transportation are required.
