Family Case Management![]() Family Outreach Center's Family Case Management Program is designed to address the counseling needs of the children and adolescents with severe behavioral and emotional dysfunctions. Referral of children and their families that fit this protocol are made to Family Outreach Center by Network 180. The children referred may also be court or delinquent wards who are involved with the 17th District Court - Family Division, under a legal guardianship with a relative, in kinship care, or residential services. Nuclear families, kinship care families, and single heads of households involved in the treatment experience are most likely impacted by:
Objective: Reduce the number of children and adolescents with severe behavioral and emotional problems entering residential treatment or psychiatric hospitals. Eligibility: Children, adolescents, and families residing in Kent County. Funding: Funding is provided by Network 180, the Heart of West Michigan United Way, and the generosity of corporate donors and private individuals.
